Who is responsible for your information?
Spark Rehab is operated by Keith Thresh in Hull, East Yorkshire. For privacy questions, call 07805 812420 or email hello@sparkrehabhull.co.uk.
Information we may receive
The enquiry form collects your name, contact details, preferred reply method, who the support is for, the best time to reply and your brief enquiry. The form is delivered by FormSubmit, which forwards it to Spark Rehab's business email address. The free guide needs no sign-up. If you contact Keith by telephone, text or email, you may choose to provide similar information.
Please do not use the enquiry form or ordinary email to send medical records, medication details or other sensitive health information. A brief outline is enough. If specialist sessions open following qualification and insurance confirmation, client assessment, consent and record-keeping will be covered by separate documents.
Why we use information
Contact information is used to answer your enquiry, arrange a conversation and take steps you request before any service agreement. Where you choose to share health information, it is used only to understand and respond to your enquiry, with explicit consent where required. Spark Rehab does not sell personal information.
Sharing and retention
Information may be handled by trusted service providers where necessary to run the website and communications, or disclosed where the law requires it. FormSubmit states that it retains form submissions for 30 days. The copy delivered to Spark Rehab's business email is normally deleted within 12 months after the last contact, unless it becomes part of a client relationship or must be kept for a legal reason.
Your rights
Depending on the circumstances, you may ask to access, correct, erase or restrict use of your information, object to its use, request portability or withdraw consent. Contact Keith using the details above. You may also raise concerns with the Information Commissioner’s Office.
This website and its contact details are not an emergency service. Call 999 for new or suspected stroke symptoms.
